The following documents are needed:

Application Form

This form provides us with most of the information needed to evaluate an applicant's suitability for the Program, and should be submitted, by mail, as early as possible. We will pay your application fee, if materials are mailed directly to our office. Simply complete and return the form along with several the supporting documents.

If you are mailing your application packet from outside the United States please have all application materials in one packet. Letters of recommendation and transcripts should be sealed in a separate envelope within the main application packet.

Personal Statement

A statement several pages in length should be returned with the application form. It should be articulate and give evidence of a strong commitment to research. Please include the following information:

1. A description of your present academic interests and long-range career goals.

2. Reasons for applying to this and other programs.

3. A description of your scientific interests and a list of faculty whose research interests are compatible with your own. (Although it is not essential to have specific research interests at this point, it is important that students admitted to the program have several potential research advisors.)

4. A summary of previous experience including laboratory research.

GRE Scores

To avoid possible delays, it is advisable to include a photocopy of your own test reports with your completed application packet.

If you are accepted you will need to arrange to have an official copy of your scores sent to the Program address below. These scores will need to be valid for the beginning of the Fall semester. The institution code is 4853 and the departmental code is 0202.

TOEFL Scores

To avoid possible delays, it is advisable to include a photocopy of your own test reports with your completed application packet.

If you are accepted you will need to arrange to have an official copy of your scores sent to the Program address below. These scores will need to be valid for the beginning of the Fall semester. The institution code is 4853 and the departmental code is 74.

Transcripts

Arrange to have one official transcript from each institution you have attended. To avoid possible delays, it is also advisable to include photocopies of important transcripts with the completed application form.

Letters of Recommendation

We require recommendations from three persons who are in a position to comment on your probable success as a graduate student. Since many faculty are often slow to submit such letters, you may wish to ask for letters from more than three people. It is also wise to check back with reference persons to be sure that the letters have been sent. Provide each respondent with a copy of the personal reference form.
